Be sure you fill this out completely each time you drive the vehicle.  If you travel to multiple places in the same trip, please log each destination so when we compare mileage, etc. we can make sense of everything.  If we see something out of the ordinary, we will contact you for clarification.

These are only to be used for fuel purchases and car washes at authorized stations – see
attached instructions for use of WEX Card.

Driver Prompt ID’s: Used like a PIN – Authorized drivers are to obtain an individual PIN# to be
used for fuel purchases. This means if you drive any of the AOC Fleet you must have your own
PIN#. Once you have taken the Defensive Driving Course you should obtain a PIN# so you will
be ready when you need to drive a vehicle. PIN#’s must NEVER be shared. If you are out on the
road and your PIN# does not work, you have one of three options: 1) If your PIN# doesn’t work
at the pump, you can try going inside to see if it works in there; 2) you can try another gas
station; or 3) pay for the gas yourself, save the receipt and submit for reimbursement with your
travel request. If you need a PIN# or need your PIN# reset, please contact AOC Purchasing and
we can assist you. As with passwords, you must keep your PIN# secured at all times.

“Only authorized passengers may occupy a state vehicle” – this means state employees only
(no family member, friend, or pets!).

Only individuals who have a current Defensive Driving Certificate and are set up with the WEX
account are authorized to Drive the AOC Fleet.

Please remember to obtain a receipt after utilizing the WEX card – if the pump does not supply you with
a receipt, it is your responsibility to obtain a copy from inside the gas station.

If you are not able to obtain a receipt you will need to complete a Lost Receipt Affidavit with the Agency
Director’s signature. This form will be turned in in place of the receipt for invoicing.

Please ensure you have retrieved all personal items from the vehicle, remove any trash – if the car is in
need of a clean (exterior wash, vacuum) please ensure it is done. Do not return the car with less than a
half a tank of gas.

If you are in an accident, even if it is a “fender-bender” or something minor, you are REQUIRED to submit a police report.  You must also contact FSD as soon as possible after this occurs.  We will then determine how/where to fix the vehicle.  Also, if something breaks in the vehicle while you are driving it, or (as an example) you accidentally scratch something while parking, PLEASE DO NOT BE AFRAID to contact FSD about it.  Some of these vehicles are older and things break, or other things “happen” and can be fixed easily if we know about them.  Please don’t leave the car in a possible “unsafe” condition for the next person – because that person could be YOU!
